So that you are prepared for your first visit with Dr. Hannon and her team, we have provided the necessary forms for you to use. Many patients are familiar with HIPAA regulations and the Notice of Privacy Practices is for your records. Because relationships built on open communication and trust are important for our patients and team, by printing and signing the Acknowledgment and Consent form, you agree that we have given you the Privacy Practices and you understand that we will not use your protected information for any purpose outside of your dental care.
When you complete the Health History pages and bring them with you for your first visit, we will review your health and dental history during the initial interview. It is very helpful to have this information gathered ahead of time for you and for the dental team. Also included on these pages is space to record your basic information, emergency contact information, and dental benefit information, if you will be using a dental benefit to help cover the cost of your dental care.
